CHEROKEE – The Crawford County Health Department will be offering sports physicals for $10. They are scheduled to be at the Southeast High School (SHS) on Tuesday, May 5, from 8 AM – 3 PM and the Southeast Middle School (SMS) on Wednesday, May 6, from 8 AM – 2:00 PM.  Any student taking advantage of this opportunity must have the Parent Consent Form, the Sports Physical Form, and the Concussion Form completed and signed along with the $10 fee the day of the clinic (cash, or check made out to Crawford County Health Department). Hard copy forms will be available in the school offices, for students or parents to pick up, and can also be accessed through the links below.

We do ask that parents, who are interested in their child(ren) receiving a physical, please contact the respective school (SHS – 620.457.8365 / SMS – 620.457.8315), so a list can be made for the Crawford County Health Department.
